Living out your years with a healthy mind, body, and soul is a dream and a goal for almost everyone. But a debilitating illness, an emotional setback, or simply the aging process can make day-to-day tasks challenging and life less manageable for a loved one. So, getting them some help, to maintain their quality of life, becomes a crucial first step.

Sometimes it’s a sensitive topic, especially if your loved one is in denial and has a fear of losing valued independence – we understand this. Families may also feel scared, guilty, helpless and directly responsible for the safety and well-being of a loved one. But the increasing demands on their own personal and professional lives leave limited time for sick relatives. We empathize and understand this too. Hence for such people, homecare is the best care a family can provide.

What is homecare?

At its basic level, “homecare” means exactly what it sounds like – medical care provided in a patient’s home.  It can also be defined as a wide range of health care services that can be provided in your home for an illness or injury.

What is Outpost homecare?

The Outpost homecare service connects you to the largest network of homecare nurses in Nigeria. With this service, you can get the care you need for you or your loved ones from the comfort of your home. All services rendered by accredited registered nurses (Nurses are accredited by the Nursing and Midwifery Council of Nigeria and are trained to give homecare)

Services you get with the Outpost homecare:

The Outpost homecare includes

1. Chronic Disease Care

We offer an integrated care approach to managing illness and symptoms of a long-term disease, thereby allowing patients to enjoy life. From diabetes to high blood pressure or stroke. Chronic disease care includes screenings, check-ups, monitoring and coordinating treatment, and patient education. It can improve quality of life while reducing the health care costs if you have a chronic disease by preventing or minimizing the effects of a disease.

2. Medication delivery + Administration

The administration of a medicine is a common but important clinical procedure hence the need for it to be taken seriously.

The manner in which a medicine is administered will determine to some extent to which a patient gains any clinical benefit, or whether they suffer any adverse effect from their medicines. For a sick loved one, homecare nurses will effectively deliver a pharmacist prescription or doctors instruction significantly improving the patient's experience of drug treatment

3. Physiotherapy

Physical therapy addresses illnesses or injuries that limit a person's abilities to move and perform functional activities in their daily lives.

Physiotherapists can establish a management plan and arrive at a diagnosis using an individual's history and physical examination and also when necessary, incorporate the results of laboratory and imaging studies like X-rays, CT-scan, or MRI findings

4. Wound care

Wound care plays an important role in recovery as proper would care prevents infection and other complications and helps speed up the healing process with less scarring.

By keeping continual attention on the wound dressings and bandages the risk for infection and other complications is greatly decreased. A health professional can make medically important decisions through changing the dressings, noting the wound’s progress, as well as by making observations of bleeding, temperature, discharge and smell

5. Post-Surgery care

Postoperative care is the care you receive after a surgical procedure. The type of postoperative care you need depends on the type of surgery you have, as well as your health history. It often includes pain management and wound care.

A homecare nurse can help tend your wounds, prepare food, keep you clean, and support you while you move around

6. Elderly care

Care at home is mostly needed for old people as they are more vulnerable. We provide the required extra care to remain in your homes, reducing the burden otherwise shouldered by health care facilities, assisted living centers and nursing homes.

7. Disability care

Disability care at home is simply practical help with anything from everyday tasks like housework and bathing, to assistance with complex needs. We link you up with specially trained disability carers who come into the home and lend a hand.

Good care can support function and help people affected by disability to live independently and with dignity. Each disabled person has different challenges and needs, so there can’t be an one size fits all formula. Out homecare for disabled adults requires careful assessment to work out what support is needed, and to find the right person to provide that care.

8. Post birth care

The post birth stage generally refers to the first 6 weeks after childbirth. This is a particularly vulnerable time for both new mothers and babies, which is why it is important to obtain thorough medical care during this time as new moms are at risk of serious and sometimes life-threatening health complications.

We help you effectively make a postpartum care plan and connect you to caregivers to provide superior postpartum care services can protect both the new mother and the baby

9. Digital Health Records

We embrace digital capabilities to drive connectivity to make healthcare more precise, efficient and personalized for your loved ones. You can have your digital health records updated and accessed when you change hospital and also monitor and track your vitals with Outpost
